As part of this form, you will be required to fill out the following information:

  • Credit Card Details
  • Loan Account

Details of Personal Representative(s):

  • Grant of Probate / Letters of Administration / Certificate of Confirmation
  • Name of 1st Representative
  • Date of Birth
  • Address
  • Telephone Number
  • Relationship to the Deceased
  • Nationality
  • Name of 2nd Representative
  • Date of Birth
  • Address
  • Telephone Number
  • Relationship to the Deceased
  • Nationality

Details of the account you would like the funds to be paid to:

  • Payee Name
  • Bank Name
  • Account Number
  • Sort Code

What happens next?

After you have notified Co-Operative Bank Mortgages, you should be sent a letter confirming in writing what further steps may need to take place, including any additional documentation they may need. 

They may also apply the following steps:

  • Freeze accounts until account closure
  • Cancel any standing orders and/or direct debits
