What you need
- Date of Death
- Full name and usual address
- Death certificate or interim death certificate (scan or photograph)
- A piece of photographic ID, such as a passport of driving licence, to upload and verify your identity. (scan or photograph)
How to Report a Death to MMC Pension Fund
You can register a death with MMC Pension Fund in several ways:

What information is required to tell MMC Pension Fund of a bereavement?
MMC Pension Fund asks that you provide the following information:
- Member’s pension scheme number and user name
- Their National Insurance number
- Date of Death and personal details
- The name and contact details of the person dealing with the death – don’t forget, if this isn’t you, that you will need to receive their permission to be able to pass on their details
- If possible, any information you have regarding anyone who was financially dependent upon them at the time of their death, for example a spouse or civil partner, or children

Once notified, MMC Pension Fund will talk you through what the next steps are, and if they require any further information.
